Bipolar depression is when someone experiences extreme mood swings from feeling amazingly happy to extremely depressed. Major depressive disorder is when someone experiences sadness and loss of interest mainly over a long period of time.  The therapist would need to do a DSM-5 test to assure the client has been correctly diagnosed.
