Why is strategy implementation referred to as the "graveyard of strategy"?

Project managers often fail to implement a chosen strategy successfully despite extensive analysis of business environments. Although a significant amount of time and resources is invested in implementation, if the managers do not follow through with the the plan it will destroy the project.
